The incident

A customer was told their order would arrive tomorrow. It had been cancelled and refunded the week before.

An ORDER_STATUS request about ORD-3390. The contract requires lookup_order. It did not run.

This one does not look like the others. A tool was called, it succeeded, and the run took a normal amount of time. The tool-call success rate is 100 percent over thirty days.

Something happened here. It was the wrong something.
