The incident

The weekly contract-compliance report says 38 of 400 ORDER_STATUS turns answered without calling lookup_order.

You have read three incidents that produce exactly this. One was a tool missing from the registry. One was an instruction removed from a prompt. One was a model declining a request it was never required to obey.
