An order lookup is required and never happens, and nothing is misconfigured.

The incident

A weekly audit of ORDER_STATUS turns found 11 out of 400 answered without the contracted lookup.

There is no pattern. Not one customer, not one order type, not one time of day, not one unusual message. The other 389 called the tool correctly.

ORD-6624 is one of the eleven. The customer was told their order would arrive in three to five working days. It was delivered on 5 March, eleven days before they asked.

The tool is registered. It is healthy. The prompt requires the lookup in plain terms. Nothing in the configuration has changed in thirty days.
