The incident

A customer placed one order. Their statement shows two charges of $49.99, seconds apart — and they noticed before we did.

Order ORD-1001 reads CONFIRMED in the order service, exactly once, and every dashboard is green. So two things are true at once: the order succeeded once, and the card was charged twice. That is the whole of what is known — nobody has looked at the payment provider yet.
